When something goes wrong on site, the pressure to find answers is immediate. But rushing the investigation or blaming the nearest worker does not prevent the next incident. Proper investigation gets to the root cause. That is where lasting change starts.
At Safe Metrics, we use the ICAM methodology for incident investigations. ICAM stands for Incident Cause Analysis Method. It is a structured, evidence based approach used widely across Australian industries including mining, construction and energy.
ICAM looks beyond the immediate actions that led to an incident. It examines the task and environmental conditions, the individual and team factors, and critically the organisational factors that allowed the conditions for the incident to exist.
We collect evidence through site inspections, witness interviews, document reviews and timeline analysis. The findings are organised into a clear report that identifies contributing factors at every level and recommends targeted corrective actions.
Not every incident needs a full ICAM investigation. Minor first aid events might only need a basic review. But serious injuries, dangerous occurrences, near misses with high potential and notifiable incidents all warrant a thorough investigation.
We can also investigate patterns. If your risk register shows repeated incidents in the same area or task, a deeper investigation can uncover systemic issues that simple corrective actions have not fixed.
For notifiable incidents, there are strict reporting obligations under WHS legislation. We help you meet those obligations and prepare documentation that satisfies the regulator. Our reports are factual, well structured and stand up to scrutiny.
After the investigation, we work with your team to implement the corrective actions and verify they are effective. Because an investigation report sitting in a drawer does nothing for anyone.
If your team needs support after a workplace incident, or you want to improve your investigation capability, reach out to us. We also offer workplace audits that can identify risks before they become incidents.
While we conduct investigations directly for clients, we also run ICAM investigation training for safety professionals, managers and supervisors who want to build their own capability. Having trained investigators in your team means minor incidents can be investigated promptly without waiting for external consultants.
We train participants in the ICAM methodology, evidence collection and preservation, witness interview techniques, root cause identification and corrective action development. Participants leave with practical skills they can apply immediately.
For high consequence industries like mining, oil and gas, construction and rail, having multiple qualified internal investigators is not optional. Regulators expect it. Major incidents will be investigated whether you are ready or not. Having trained people on your team makes a difficult situation more manageable.
We also offer investigation report review services. If your team has completed an investigation and you want an independent check before the report goes to the regulator or board, we review it for completeness, accuracy and the robustness of the corrective actions recommended.
ICAM stands for Incident Cause Analysis Method. It is a structured approach to incident investigation that examines contributing factors at four levels: the absent or failed defences that should have prevented the incident, individual and team actions, task and environmental conditions, and the organisational factors that allowed the conditions to exist. It goes deeper than traditional investigations that focus only on immediate causes.
Serious injuries, fatalities, dangerous occurrences and high potential near misses warrant full ICAM investigations. For notifiable incidents, regulators may conduct their own investigation. Having your own thorough investigation completed promptly demonstrates due diligence and helps with corrective action planning regardless of the regulatory outcome.
Under the WHS Act, PCBUs must notify the relevant regulator immediately upon becoming aware of a death, serious injury or illness, or a dangerous occurrence. The definition of notifiable incidents varies slightly across jurisdictions. We recommend checking the requirements for your state with your legal team and contacting us for investigation support as soon as practical.
A straightforward investigation of a single incident typically takes 1 to 2 weeks including evidence collection, interviews and report writing. Complex incidents involving multiple contributing factors, significant physical evidence or multiple witnesses take longer. We can provide preliminary findings within days if the situation requires it.
Absolutely. Near misses with high potential consequences deserve the same investigation rigour as actual incidents. The outcome was different only by chance. Investigating high potential near misses and acting on findings prevents future incidents. Businesses that only investigate after harm occurs miss the most valuable learning opportunities.
